31.4. A member who, in accordance with the second paragraph of section 60.5 of the Professional Code (chapter C-26), denies the client access to the information contained in a record established in the client’s respect must specify to the client, in writing, the reasons for his or her refusal and enter the reasons in the record.

31.4. A member who, pursuant to the second paragraph of section 60.5 of the Professional Code (chapter C-26), denies the client access to the information contained in a record established in his respect shall inform the client in writing that the disclosure would likely cause serious harm to the client or to a third party.
The member must also, in the same document:
(1)  identify the serious harm to the client or to a third party;
(2)  identify the third party referred to.
